http://www.gw2build.com/builds/simulator.php#1.3.11.12.0.0.0.0.0.0.54.74.72.73.78.10.1.16.0.0.0.0.0.0.181.190.192.198.202.205.0.0.0.5.0.30.30.5

I run a 5/0/30/30/5 build with Hammer and Great Sword.
Hammer is absolutely necessary, but Great Sword is interchangeable with Mace/Shield or Mace/Focus if you want more defense.
I built this character around being a pure tank. Sure I carry a scepter for those moments you need to range, but in all honesty you dont really need it. All of your utilities heal, and all the symbols you make heal you and your allies. This makes for excellent stacking. The 5 points in power is for a symbol (yes it heals) that automatically gets dropped when your health falls too low. I put the 5 points into virtues for the added boons from using my virtues. The items in that build calculator dont really matter since i have ascended gear, but I do use Runes of the Grove.

If you want the tankiest tank that can tank a tank, this build is for you. Twisted Evil

Personally I use Runes of the Grove and focus my armor oh healing power and toughness, and then a mix of power and vitality for the other stats. my accessories are currently sapphire, exotics, and then I have an ascended Prayer to the Otter amulet.
