crafting an ascended back item, and general notes on ascended items
read:
http://wiki.guildwars2.com/wiki/List_of_ascended_equipment
Or the below
To craft an ascended back item:
you need either:
1850 fractal relics, a vial of condensed mists essence (obtained from fractals of level 1-9), 24 skillpoints
The general rule here is to buy the fractal capacitor of desired stats from the fractal vendor, then combine it in the mystic forge with either 240 philosopher's stones (for a defensive infusion slot) or 40 crystals (for an offensive infusion slot). (The philosopher's stone / crystals are bought from the NPC named miani near the mystic forge.) along with a vial of condensed mists essence and a "gift of ascension" (bought also from fractal vendors). The differences between offensive and defensive infusion slots will be explained later in the passage.
OR:
50 globs of ectoplasm, a vial of condensed mists essence, 24 skillpoints and 250 of a certain T6 mat (blood or fang or scale or bone or claw)
The general rule here is to combine the vial of condensed mist essence, 50 globs of ectoplasm and the 250 T6 mats (blood for power/precision/crit damage stats, fang for power/toughness/vitality, scale for power/toughness/crit damage, bones for precision/toughness/condition damage, claw for power/toughness/vitality/magic find), and 240 philosopher's stone (for a quiver which has an offensive infusion slot) or 40 crystals (for a book which has a defensive infusion slot).
Infusions and infusion slots:
Other than the infusion which can be bought from the fractal vendors for 75 relics (the versatile simple infusion), there're a lot of different infusions that can be made by the player by combining materials in the mystic forge. Some of these are called defensive infusions, some are called offensive infusions. These infusions offer a slight boost in a certain stat in addition to the 5 agony resist.
Defensive infusions have 3 types:
1: Healing infusion (+5 healing) (made in the mystic forge by combining 250 karka shells, 100 T6 totems, 1 Eldritch scroll (bought from miani again), and 20 mystic coins)
2: Resilient infusion (+5 toughness) (made in the mystic forge by combining 250 karka shells, 100 T6 scales, 50 Philosopher's stone (bought from miani again), and 20 mystic coins)
3: Vital infusion (+5 vitality) (made in the mystic forge by combining 250 karka shells, 100 T6 bones, 50 Philosopher's stone (bought from miani again), and 20 mystic coins)
Offensiver infusions have 3 types:
1: Malign infusion (+5 condition damage) (made in the mystic forge by combining 250 passion fruits, 100 T6 venom sacs, 50 philosopher's stone (bought from miani again), and 20 mystic coins)
2: Mighty infusion (+5 might) (made in the mystic forge by combining 250 passion fruits, 100 T6 blood, 50 philosoher's stone (bought from miani again), and 20 mystic coins)
3: Precise infusion (+5 precision) (made in the mystic forge by combining 250 passion fruits, 100 T6 claws, 50 philosopher's stone(bought from miani again), and 20 mystic coins)
So, here is how it works: defensive infusion slots can only be upgraded by a defensive infusion or an "Omni infusion" (discussed later), and an offensive slot can only be upgraded by an offensive infusion or an "Omni infusion".
So what are Omni infusions?
Omni infusions have "versatile" in front of their name, and can be used on both offensive and defensive infusion slots.
There're 7 types of Omni infusions currently:
1: Versatile Malign infusion (made in the mystic forge by combining 100 passion flowers, 2 Eldritch scrolls, 50 mystic coins, and 250 of T6 venom sacs)
2: Versatile Mighty infusion (made in the mystic forge by combining 100 passion flowers, 2 Eldritch scrolls, 50 mystic coins, and 250 of T6 blood)
3: Versatile Precise infusion (made in the mystic forge by combining 100 passion flowers, 2 Eldritch scrolls, 50 mystic coins, and 250 of T6 claws)
4: Versatile Healing infusion (made in the mystic forge by combining 100 passion flowers, 2 Eldritch scrolls, 50 mystic coins, and 250 of T6 totems)
5: Versatile Resilient infusion (made in the mystic forge by combining 100 passion flowers, 100 philosopher's stones , 50 mystic coins, and 250 of T6 scales)
6: Versatile Vital infusion (made in the mystic forge by combining 100 passion flowers, 2 Eldritch scrolls, 50 mystic coins, and 250 of T6 bones)
7: Versatile Simple infusion (bought from fractal vendors for 75 fractal relics)
One quite important note:
Ascended items (rings in particular) have "unique" in their item description (below the line "Required level: 80"). This means that one cannot equip two pieces of items which have the same name.
Take for example: The "royal signet of doric" grants power/toughness/vitality, and has a defensive infusion slot. But we cannot equip two at a time, so a viable method is to equip another ring of similar stats (eg the "Lost Seal of Usoku" which grants power/toughness/vitality, and has an Offensive infusion slot).
Note that infused rings (eg "Royal Signet of Doric (Infused)") doesn't count as the same item as the non-infused ones (i.e "Royal Signet of Doric", in this case), so one can equip the two rings together. (but the infused ring gives +10 agony resist with the infusion implemented, while the non-infused one only gives 5)

Re: crafting an ascended back item, and general notes on ascended items
You get 20 from the jade maw chest, which appears in every even level of fractals (2/4/6/. There is also a daily chest if you do either one of 2/4/6/8 daily, which gives an additional 20 relics.
So by doing level 2 fractals daily, you'll get at least 50 relics a day
